HomeSafe understands that time is of the essence when it comes to your residential placement needs.

To help streamline the process and ensure timely and high-quality service, HomeSafe has a dedicated email address (Referrals@helphomesafe.org) for referrals to our Residential Group Care program – Specialized Therapeutic Group Care (STGC).

Our Clinical Directors and Referral Coordinator will work closely with you to expedite the process, all while ensuring full attention is given to the individualized needs of youth.

Getting Started

HomeSafe’s Complete Referral Packet (download)
Please fully complete the form to allow for thorough review of the youth’s needs.

Gather additional documentation needed to complete a referral application, including:
1.) Recent suitability assessment or CSST staffing that recommends STGC level of care
2.) Current Psychiatric Evaluation and/or Assessment (youth must meet clinical criteria for placement, including having a primary DSM-5 Axis I diagnosis)
3.) Comprehensive Behavioral Health Assessment (CBHA) or Bio-psychosocial history
4.) Current Treatment summary/discharge paperwork
5.) Psychological Testing with IQ Score

Step 1 – Submit Referral Packet and supporting Documentation:
To submit a referral, please email required information to Referrals@helphomesafe.org or fax to 561-383-9851. The Referral Team will review within 24-48 hours. You will then be contacted to discuss the referral and, if appropriate, schedule an interview the youth.

Step 2 – Interview
For optimum placement and treatment planning, it is beneficial to conduct a face-to-face interview with the youth prior to admission. This serves as an opportunity for HomeSafe staff to learn more about the youth, as well as the youth to learn more about the program and expectations. In-person interviews are preferred; however, remote interviews can be completed via Skype (when necessary).

Step 3 – Admission
A decision will be made within 24 hours after the interview. Once acceptance notification is provided, you will work collaboratively with HomeSafe staff to ensure all required documents are submitted. An admission date will be then be confirmed upon all documents being properly processed.

Important Information
The legal guardian will need to sign admission documents at the time of admission. The youth will need to bring two weeks of clothing and all medications (At least 5 days worth of medication must be brought with youth at admission.)
*Linens and hygiene items are supplied by the program.

Exclusionary Criteria
Unfortunately, HomeSafe is unable to admit youth with any of the following:
1) Developmental or cognitive delays (IQ must be above 70)
2) Complex medical conditions requiring care beyond routine appointments
3) Pregnancy or has a child of their own
4) Youth unable to share a room (all programs have 2 children per bedroom/limited single rooms)
